We’ve two suggestions, then again:
1) #5 in addition to #17 really depend one particular and the subject matter of it, and the option wasn’t there to become picked.
2) You should have some sort of privacy policy merely so people know you aren’t selling the information. I know you’re not since it’s just a San Diego university project page, but people are certainly not always as aware as My organization is.

A totally feedback. You aren’t going to learn much coming from a survey like the following. You would learn additional from a mental health analysis of different media given to your audience.
As an illustration, create two pages: one with your flash intro this " loads" indefinitely, all the while timing people’s side effects (how quickly many people close the window) plus generate your effects from that.
My basis due to this suggestion is that what folks SAY they will do and what many people ACTUALLY do are rarely identical.
